Hot Lunch
Hot Lunch Order Form (Jan-May)
Please read the following information carefully:
This year, Frost PTA will be offering hot lunch on a PRE-ORDER basis only. There will be NO CASH SALES accepted the day of hot lunch.
Each lunch will include a bottle of water and an optional flavor packet. (Such as Gatorade, Crystal Light, etc)
Frost PTA is not responsible for food allergies. Some lunches may contain or have contact with nuts, gluten, soy, pork etc. The local restaurant we order from is listed. Parents are encouraged to call and check on ingredients and or concerns.
Frost PTA cannot give refunds if you child is absent on a day you have ordered hot lunch. You have the option to call the office and notify them you will be picking up your child’s lunch.
The order form through the end of December is attached. Another order form will be coming home in December for 2012.
Please make sure that your child’s name and lunch period are clearly marked on the order form.
Make sure to print a copy of the order form for your records.
Order forms along with a check payable to Frost PTA are due by Friday, September 2nd.
